چکیده

context functional humeral bracing remains the gold standard for treatment of humeral shaft fractures. there is an increasing trend in the literature to perform operative fixation of these fractures. evidence acquisition the aim of this systematic review was to compare the level one evidence for the outcome of non-operative with operative management of humeral shaft fractures in adults. a comprehensive electronic literature search of medline and pubmed was performed with specific inclusion criteria to identify randomized controlled trials. results in total, seventeen different studies were identified from the search terms and combinations used. only one study met the inclusion criteria; however, this was a published study protocol of an ongoing trial currently being conducted. one additional published protocol for an ongoing trial was also identified, but this was for a prospective comparative observational study. although this latter study may not be level one evidence, it would offer great insight into the functional outcome of humeral shaft fractures and economic implications of operative management, which is currently not addressed in the literature. two retrospective comparative studies were also identified, one of which demonstrated a significantly lower rate of non:union: and mal:union: in those patients undergoing operative management. conclusions this systematic review demonstrated a deficiency in the current literature of level one evidence available for the management of humeral shaft fractures. the current ongoing randomized control trail would offer a greater insight into the management of humeral shaft fractures and help confirm or refute the current literature. if this randomized control trial affirms the reduction in the rate of non:union: with operative fixation, a cost economic analysis is essential. as it would seem to offer operative management to all patients may be over treatment and not to offer this at all would undertreat.

Humeral shaft fractures: retrospective results of non-operative and operative treatment of 186 patients.

BACKGROUND Humeral shaft fractures account for 1-3% of all fractures and 20% of the fractures involving the humerus. The aim of the current study was to compare the outcome after operative and non-operative treatment of humeral shaft fractures, by comparing the time to radiological union and the rates of delayed union and complications. METHODS All patients aged 16 years or over treated for a...

HUMeral Shaft Fractures: MEasuring Recovery after Operative versus Non-operative Treatment (HUMMER): a multicenter comparative observational study

BACKGROUND Fractures of the humeral shaft are associated with a profound temporary (and in the elderly sometimes even permanent) impairment of independence and quality of life. These fractures can be treated operatively or non-operatively, but the optimal tailored treatment is an unresolved problem. As no high-quality comparative randomized or observational studies are available, a recent Cochr...

Proximal Humeral Fractures: Nonoperative Versus Operative Treatment

Background: Management of displaced proximal humeral fractures is subject of ongoing debate. Objectives: We aimed to review our results of operative treatment of proximal humeral fractures compared to age-, sex, and fracture-type controlled conservative treatment. We hypothesized that there is no significant difference in upper-extremity specific disability between patients treated with operati...

Operative treatment of humeral shaft fractures. The Leuven experience.

For many years, plating has proved to be a reliable method for the fixation of fractures of the humeral shaft. In the early nineties however, intramedullary devices became increasingly popular for fractures of the humeral shaft. This was based on a global tendency towards minimally invasive surgery, and the attractiveness of the relatively simple procedure and potentially lower complication rat...

proximal humeral fractures: nonoperative versus operative treatment

results at follow-up, the nonoperatively treated patients had better functional outcomes than the operatively treated patients. nonoperatively treated patients also scored better on pain intensity and satisfaction. there were no significant differences in cesd, pcs, and sf-36 physical- and mental health summary scores between cohorts. conclusions the results of this study suggest that operative...
